Our forms are available as PDF (Adobe Acrobat) files. To download or view any form, please click on the link. The Membership Application, Jump Harness Waiver, Medical Release Waiver and Contract forms may be filled out on your computer and printed. Please note that these files cannot be saved to your hard drive, so be sure to use the print function before you close the file, or all information on the forms will need to be re-entered.

The 2010 Fall Contract is here. Please fill out the pdf form on your computer, print, sign and send to the PSC office.
Apologies for the quick turnaround time on this, but it was worth the wait because confirming that the north rink is going back a few weeks earlier this year than in the past allows us to put out a full contract through March. Thank you all so much for your patience with this! Some important points to note:
August 22 – CHANGE FEE GOES INTO EFFECT: $15 change fee will be assessed for changes made to a contract after this date.
August 29 is an important date to note because this is the last day you can add sessions to your fall contract at the $10 contract rate. After that date, you can reserve sessions, but it will be at the walk on rate.
August 29 – DEADLINE FOR LEVEL DESIGNATIONS TO BE MAINTAINED. Any session without 10 contracted skaters as of Aug. 29, 2010, will be designated OPEN.
Missed Sessions – IMPORTANT CHANGE. Skaters who miss a contracted session will be given a voucher that will be good for 30 days from the time of the missed session to be used as a make up. These are not transferrable outside of the skater’s immediate family, and this replaces “make-up coupons”. We hope this will be an easy way to handle missed sessions, and still get credit for your contracted time.
The test level designations for each session refer to freestyle level passed.
Please be sure that we have a current email address as this is our primary method of communication.
PSC maintains it's policy not to prorate sessions. There will be no RETROACTIVE refunds for contracted sessions. Short term absences, illnesses or injuries for which a skater misses less than two weeks (with or without a doctor’s note) are not refundable. Make up vouchers will be issued.
